AI intake, with the form still in it

Intake that listens.Triage that keeps up.

Urai Forms helps people tell you their whole story, reads every answer and attachment, and puts each response on a board your team works through.

Where will the work take place?

Moncton for now. We might add a site in Ontario next spring, but it isn't confirmed yet.

I've put Moncton as your primary site and noted Ontario as possible. Have a look before you submit.

Who is planning work in Ontario?

Ada Okafor lists Ontario as a possible second site from next spring. Her scope of work names the same site.

Forms weren't built for real situations.

Intake is where a team first learns about the person in front of it: a client, an applicant, a partner. It's usually done with a form, and forms fail in three ways.

  1. 01

    Context gets flattened

A person's situation is squeezed into dropdowns and checkboxes. The detail that decides what happens next doesn't fit.

  2. 02

    Uncertainty gets punished

“I'm not sure” has nowhere to go, so people guess or leave. Often they're the ones with the most to tell you.

  3. 03

    Triage waits for a human

Someone still opens every response, reads every attachment and decides who handles it before anything moves.

The cost isn't only people who never finish. It's decisions made on half the story, after a week in someone's inbox.

The form is the record. The conversation is the help.

Replacing forms with a chat trades one problem for another: a transcript you can't filter, chart or compare across a thousand people. Urai Forms keeps the structure, and puts the conversation where it's needed.

  • Beside the person answering, so they can explain rather than guess
  • Beside the team reviewing, so every answer is understood
  • With a record underneath that stays structured throughout

You don't have to choose between nuance and structure.

  1. Conversation

People answer in their own words

  2. Structured answers

Validated fields you can filter, chart and export

  3. Board

Every person tracked to a decision

Understand first. Then triage.

Three things happen to every response, in this order.

Intake people finish

Help is right beside the form, and people move through it at their own pace.

  • An assistant fills in the draft as people talk. Only they can submit.
  • Only the questions that apply, based on earlier answers
  • A personal link and an autosaved draft to come back to

Every answer understood

Answers and attachments are read for you, so nothing waits in an unopened file.

  • Ask about a single response, with sources cited
  • Ask across every response at once
  • Search what people wrote, not just form titles

Triage that keeps up

Everyone you invite is on the board from the start, not just the people who finish.

  • Cards move on their own as people start and submit
  • Then your team works them through its own stages
  • Several intake forms can feed one pipeline

Built for intake that starts a process.

Not one-off surveys, but moments where the answers decide who handles what next, and each person is followed from invitation to decision.

  • Client intake
  • Partner and vendor questionnaires
  • Applications
  • Research collaborations
  • Contractor onboarding
  • Eligibility checks

FAQ

Questions people ask

Is this a chatbot instead of a form?

No. The form stays, because structured answers are what your team filters, charts and exports. The assistant sits beside it and helps people answer. They can fill the form in themselves, talk it through, or do a bit of both.

Does the AI submit on someone's behalf?

Never. It can fill in a draft when asked, every word of it is on screen to read and change, and submitting stays with the person whose answers they are.

Can people stop and come back later?

Yes. Each person gets their own link, and their draft saves as they go. They can close the tab and pick up where they left off until you close submissions.

What happens to the files people upload?

They're read along with the answers, so your team can ask about a CV or a scope of work and get an answer that points to the page it came from.

How does triage work?

Everyone you invite becomes a card on a board. Cards move by themselves from invited to in progress to submitted. After that, your team moves each one through the stages you define.

If a form starts a process, it should understand first.

Move one intake onto Urai Forms and see how much sooner your team knows what to do next.
